Periodontal ve Peri-implant Plastik Cerrahi: Temel Kavramlar ve Yenilikçi Yaklaşımlar

Periodontal rekonstrüktif cerrahi alanında elde edilen yeni gelişmeler klinisyenler için minimal invaziv, hasta dostu yaklaşımların öne çıktığı yeni bir dönemi başlatmıştır. Periodontal sorunlu dişlerin kurtarılması amacıyla gerçekleştirilen rejeneratif periodontal cerrahi işlemlerin başarısı, son 20 yılda yapılan bilimsel çalışmalarla ortaya konulmuştur. Yenilikçi cerrahi teknikler ve sofistike flep tasarımlarıyla birlikte daha iyi klinik sonuçlar elde edilirken, hasta sıkıntıları en aza indirilmektedir. Ancak, doğal dişlerin kurtarılması her zaman mümkün olmamaktadır ve eksik dişler bölgesi için en iyi alternatif, dental implantlardır. Doğal dişlerin dental implantlarla birlikte bulunması ve iki yapı arasındaki belirgin anatomik farklılıklar tedavi yöntemlerini zorlaştırmaktadır. Kanlanma, yumuşak doku manipülasyonları, flebin kalınlığı, tasarımı ve gerilimine dikkat edilmelidir. Bu faktörlerin cerrahi sırasında ayrıntılı biçimde değerlendirilmesi gereklidir. Bütün bu kritik faktörlerin kontrol edilmesine rağmen, yapılan yanlış tedaviler veya klinik koşullar mükemmeli arayan klinisyenler için zorlu görevleri de beraberinde getirmektedir. Klinik başarı, sadece başarıyla sonlanan bir cerrahi değildir, mutlaka doğru protetik işlemlerle tamamlanması gerekir. Bu nedenle, implant tedavisi estetik sonuçlar elde etmek için diş benzeri bir çıkış profili ve sağlıklı yumuşak doku arayüzü yaratılmalıdır. Bu sunumda, diş ve implant çevresine uygulanan cerrahi teknikler detaylı biçimde ele alınacaktır.

Serhat Aslan, Ege Üniversitesi Diş Hekimliği Fakültesi’nden mezun olduktan sonra Ege Üniversitesi Sağlık Bilimleri Enstitüsü’nde Periodontoloji alanında doktorasını tamamlamıştır. Kendi kliniğinde Periodontoloji Uzmanı olarak çalışmaktadır. Milan Üniversitesi (İtalya), Biyomedikal, Cerrahi, Dental Bilimler Bölümü; Leuven Katolik Üniversitesi (Belçika), Periodontoloji ve Oral Miktobiyoloji Bölümü ve Porto Üniversitesi (Portekiz), Periodontoloji ve İmplantoloji uzmanlık eğitiminde misafir öğretim üyesidir. Tam papil koruma tekniğini (Entire papilla preservation) tanımlayarak rejeneratif periodontal cerrahi alanına katkıda bulunmuştur. Ulusal ve uluslararası kongrelerde davetli konuşmacı olarak yer almış, bilim ve sunum ödülleri kazanmıştır. Türk Periodontoloji Derneği’nde merkez yönetim kurulu üyesi ve sayman olarak görev yapmaktadır. Avrupa Periodontoloji Federasyonu (EFP), İtalyan Periodontoloji ve İmplantoloji Derneği (SIdP), Estetik Diş Hekimliği Akademisi Derneği (EDAD) aktif üyesidir. Periodontoloji alanında tek ve çok merkezli klinik çalışmalar yürüten Dr. Aslan, rejeneratif biyomateryaller ve farklı cerrahi tekniklerin yumuşak ve sert dokulara olan etkileri ve klinik sonuçları üzerine yoğunlaşmıştır.

Periodontal and Peri-implant Plastic Surgery: Basic Concepts and New Approaches

Latest developments in the field of periodontal recontruction lead clinicians into a new era of minimally invasive, patient-friendly approaches. Tooth retention procedures involving regenerative periodontal surgeries have been well documented over the last 20 years. Innovative techniques and sophisticated flap designs improved the clinical outcomes and decreased patient morbidity. Unfortunately, there are certain clinical conditions that require tooth extraction and the best alternative for tooth replacement is dental implant placement. When natural dentition combined with dental implants, treatment modalities become more complex due to anatomical differences. Blood supply, handling of the soft tissues, flap thickness, design and tension are key elements. These factors should be evaluated during the surgical intervention. Despite the control of these critical elements, the presence of soft and hard tissue deficiencies caused by improper therapies or clinical conditions will create some difficult tasks for the clinician in the way of seeking excellence. Not only will elegant surgery bring the successful outcomes, but also proper prosthetic procedures. Therefore, the healthy soft tissue interface and tooth-resembling emergence must be created by provisionalization and is of utmost importance when the aesthetic outcome is the primary objective. In this lecture, details of various surgical techniques around teeth and implants will be highlighted.

Dr. Serhat Aslan is a graduate of Ege University, School of Dentistry where he received his DDS degree. He attended Periodontology Program of Graduate Study offered by Institute of Health Sciences at Ege University and awarded a PhD degree at Periodontics. He maintains a private practice limited to periodontics, fixed prosthodontics and implants with focus on microsurgery, hard and soft tissue reconstruction and minimally invasive surgery.

Serhat Aslan is a Visiting Professor in the Department of Biomedical, Surgical and Dental Sciences, University of Milan, Italy and in the Department of Oral Health Sciences, Periodontology and Oral Microbiology, KU Leuven, Belgium. He is an Invited Professor in Post-graduate Program in Periodontology and Implantology, University of Porto, Portugal.

Currently, he is the board member and treasurer of Turkish Society of Periodontology. Also, he is a member of European Federation of Periodontology, Italian Society of Periodontology and Implantology and Turkish Academy of Esthetic Dentistry. Dr. Aslan is intensely involved in mono/multi-center clinical research and development in periodontology, with special emphasis to plastic-periodontal surgery, guided bone/tissue regeneration and esthetic implant therapy.
